Forms & Documents needed to get your room pass

IN ADDITION to your general school registration with the administration. The below forms need to be completed.

Please click on links below, print, fill out all forms, sign, date and email it back to preschool  preschoolforms@lecfl.com (forms are one per child), include updated physical and immunization forms for your child.

If you need help with any forms our office hours are Monday-Thursday 9:00 am- 3:45 pm and on Fridays from 9:00 am to 2:45 pm.
