REGISTRATION PROCEDURES AND FORMS

Thank you for your interest in Lubavitch Educational Center.  Below you will find the necessary information and forms to apply to all departments.  Should you have any questions regarding the registration process, please contact:

To Apply to
LUBAVITCH EDUCATIONAL CENTER:

• All applicants—new and current families—are to download and complete the following forms, one per family:
        Registration Guidelines and Application
        Credit Card Billing and Authorization Form

• All applicants—new and current families—are to download and complete the following forms, one per child:
       Medical Authorization Policy and Form

• If applying for financial assistance, please also download and complete the following form:
        Application for Financial Assistance

        Mail the completed forms with the required fees and documents to the Office of the Registrar, at the address indicated below.  Once your application has been accepted, you will receive a contract for the coming school year.

LUBAVITCH PRESCHOOL (BABY ROOM TO KINDERGARTEN ONLY)

    1. NEW STUDENTS ONLY:  Download and print the following Preschool Teacher/ Principal Recommendation Form and submit it as soon as possible to the administrative office of the school your child last attended.  No applications for new students will be considered for admission without this completed form.

LUBAVITCH PRE-1A / 1st GRADE

  1. CURRENT AND NEW STUDENTS: Please download and complete the Home Language Survey:

  2. NEW STUDENTS ONLY:  If you are registering for the first time at Lubavitch Elementary School, download, print out and complete the following document and submit it to the Lubavitch Pre-1A / 1st Grade school office.  No applications for new students will be considered for admission without this completed form.

  3. NEW STUDENTS ONLY:  Download and print the following three documents and submit them as soon as possible to the administrative office of the school your child last attended.   No applications for new students will be considered for admission without this documentation.

LUBAVITCH ELEMENTARY SCHOOL (NOT INCLUDING 1st GRADE)

  1. CURRENT AND NEW STUDENTS: Please download and complete the Home Language Survey:

  2. NEW STUDENTS ONLY:  If you are registering for the first time at Lubavitch Elementary School, download, print out and complete the following document and submit it to the Lubavitch Elementary School office.  No applications for new students will be considered for admission without this completed form.

  3. NEW STUDENTS ONLY:  Download and print the following three documents and submit them as soon as possible to the administrative office of the school your child last attended.   No applications for new students will be considered for admission without this documentation.

    For All Students Entering Grades 2-8:   (Download 2 copies, one for the Hebrew teacher and one for the General Studies teacher to complete.)

BEIS CHANA MIDDLE AND HIGH SCHOOL FOR GIRLS

  1. CURRENT AND NEW STUDENTS: Please download and complete the Home Language Survey:

Once your child has been accepted by the school Principal for the coming academic year, and all required documents and fees have been submitted, you will receive a contract for the coming school year.

KLURMAN MESIVTA

  1. NEW STUDENTS ONLY:  If you are registering for the first time at Klurman Mesivta (and your son did not attend Lubavitch Elementary School in the 6th grade), download and print the following documents. Complete the forms and fax them with the required documents to the number indicated on the forms.

Once your son has been accepted to Klurman Mesivta for the coming academic year, and all required documents and fees have been submitted, you will receive a contract for the coming school year.

YESHIVA GEDOLAH OF GREATER MIAMI RABBINICAL COLLEGE

  1. The first step in the registration process is to apply and be accepted by the Yeshiva administration as a student for the coming year.  Print out and complete the following form and fax it to the number indicated therein:

  2. Once the student has been accepted to the Yeshiva Gedolah for the coming academic year, the following forms are to be printed out, completed and submitted in a timely manner to the following address:
